Physical Examination Requirements
- A current medical physical examination is required prior to each year a student will participate in the SIJ Athletic Program (option beginning in 5th grade). Physical exam forms must be submitted to the office prior to the start of the sports program.
- A dental examination is required (by state law) upon entrance to kindergarten, 2nd and 6th grades. Dental exam forms must be submitted to the office prior to the first day of school.
- A medical physical examination is required (by state law) upon entrance to kindergarten (including a lead certificate) and again to 6th grade – or to any grade if a student has not previously submitted a physical examination form to St. Isaac Jogues. Physical exam forms must be submitted to the office prior to the first day of school.
